Patrick Mendenhall is a pilot for a major international airline and is a member of Crew Resource Management, a company originally founded to facilitate Crew Resource Management and Risk Management training for Corporate Flight Departments. Patrick began flying at age fourteen and obtained his private pilot's license at age nineteen. He graduated from the University of Washington with a Bachelor of Science degree in Aeronautics and Astronautics in 1980, and went on to work for Boeing as a flight test engineer prior to joining the navy in 1981. He spent eight years on active duty as navy fighter pilot and retired from the naval reserves. His duties included several leadership and management positions as well as a tour as an operational test pilot in the Navy's Air Test and Evaluation Squadron (VX-5) in China Lake, California. At VX-5, Patrick was involved in a wide variety of tactical and operational test and evaluation projects and research that capitalized on his interest in human factors integration in system design and operation. After leaving active military service, Patrick joined Northwest Airlines in 1989. The majority of his airline experience has been in international operations, flying "wide body" aircraft, including the 747-400/200, DC-10 and currently on the Airbus A-330. In over forty years of flying, Patrick has accumulated over 20,000 hours of flight time including 225 carrier arrested landings (with 116 at night). Patrick has also served on the boards of Security Pacific Insurance Company and J. Mading Companies.As a partner in CRM, LLC, Patrick's participation in course design and content development has been driven by a passionate interest in human factors issues as they affect human performance. He also facilitates Crew Resource Management and Threat and Error Management training to Corporate Flight Departments as well as to audiences in other High Risk industries, particularly healthcare. Patrick's interest in applications of the aviation safety model to other high risk / high reliability industries led to him join forces with two well-known patient safety advocates, Suzanne Gordon and Bonnie O'Connor, PhD, to write a book on this subject. He recently completed "Beyond the Checklist - What Else Healthcare Can Learn from Aviation Teamwork and Safety" (www.BeyondTheChecklist.com) that was released December 18, 2012. The book has been endorsed by many prominent safety advocates from both Healthcare and Aviation industries including Lucian Leape, MD and Captain Chesley Sullenberger (who wrote the Foreword for the book).Patrick has three beautiful daughters and resides in Tacoma, Washington with his wife, Martha and granddaughter, Violet.
